Please fill in one request form per pet to avoid any confusion with prescriptions. If you require a special order, fill in the 'additional notes' as well or call us for assistance. This form is for refills only and not for new prescription requests.

Please Note: Your prescription refill order needs to be approved by your veterinarian. Please allow 48 hours for your refill.  We will notify you immediately if we need further information. We will also phone you when your prescription is ready for pick up.
